Faculty

Amy Goodman

Amy Goodman is Senior Project Manager for the Clinical and Quality Partners Practice at PCDC. She is an NCQA Patient-Centered Medical Home (PCMH) certified content expert who develops PCMH training tools for PCDC staff, clients and medical residency programs, and supports practices to facilitate progress toward defined goals and timelines. She analyzes current practice workflows, policies and data to advise practices on opportunities for improvement, and leads client behavioral health integration efforts. Ms. Goodman was Practice Manager at YAI Premier HealthCare, in Manhattan, and developed and implemented the NextGen EHR training curriculum across the five YAI Premier HealthCare centers. As Coordinator of Rehabilitative Services at AHRC, she led a team of physical, occupational and speech therapists who provided care in seven outpatient clinical and day program settings. Ms. Goodman received her BA in Child Development from Michigan State University and her MSW from Wayne State University.

Sarahjane Rath, MPH, CHES, LCSW, CPC-A, PCMH CCE

Sarahjane Rath, MPH, CHES, is a Curriculum Development and Training Specialist at Primary Care Development Corporation (PCDC), both on the Clinical and Population Health team and the High Impact Prevention (HIP) in Healthcare CDC HIV Prevention team. On the Clinical and Population Health teams, Ms. Rath develops curricula to address specific needs for the Care Coordination/Management serving community. On the HIP in Healthcare team, she provides technical assistance to CDC-funded HIV testing organizations and works with health care organizations to maximize their potential in HIV testing and prevention and working with people who are HIV positive. Ms. Rath holds a BA in Community Health Education and an MPH from Rutgers University.

This live course grants 1.0 Continuing Education Unit (CEU) points for PCMH Certified Content Experts.
