IMPROVING HEALTH THROUGH COMMUNITY PARTNERSHIPS
Community-Focused Care Accreditation helps healthcare organizations cultivate and grow high-impact community-based partnerships to connect members or patients to non-medical or social needs resources that support the pursuit of their best possible health. This Accreditation is a standalone program that was developed in collaboration with healthcare and community-based stakeholders to acknowledge the unique role of non-medical or social needs—and community health—on population health outcomes.
Data-Driven Framework to Cultivate Community Partnerships
Organizations Accredited in Community-Focused Care benefit from this program’s standardized framework to align local, state and contracting partners around common goals tailored to their community’s unique needs. Accredited organizations demonstrate that they collaborate with cross-sector partners to improve overall population health by:
- Developing a robust understanding of non-medical drivers of health (e.g., upstream drivers, social needs, social determinants of health (SDOH) that impact their member or patient population and their broader community.
- Establishing mutually beneficial partnerships to connect members or patients to needed resources.
- Continuously measuring progress on their joint efforts to meet their population’s needs and to improve their screening and referral processes.
