Medical Boards Awarding Certification for Recognition

ABFM 
ABFM-certified physicians enrolled in NCQA's BPRP, DRP, HSRP and PCMH 2011 Recognition Programs are eligible to receive Part IV Maintenance of Certification credit for achieving NCQA Recognition.

ABIM 
ABIM-certified physicians enrolled in NCQA's Diabetes, Back Pain, or Heart/Stroke Recognition Programs can use the data obtained from these programs to complete the ABIM Self-Directed PIM and earn Maintenance of Certification credit.

The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.

NCQA Patient-Centered Medical Home (PCMH) Recognition is the most widely-used way to transform primary care practices into medical homes.

If no >
Order FREE online application. > Do you have 3 or more practice sites? If yes > STOP! Obtain multi-site approval. > Attend FREE Standards & Guidelines training. > Transform practice using Standards & Guidelines (3-12 months).

Attend FREE software training (at least 30 days before submitting ISS Survey Tool). > Purchase ISS Survey Tool. > Submit online application. > Prepare and submit ISS Survey Tool to NCQA. > NCQA reviews ISS Survey Tool (30–60 days). > Receive decision (results in ISS).
