Medical Boards Awarding Certification for Recognition

American Board of Family Medicine (ABFM)
ABFM-certified physicians enrolled in NCQA's DRP, HSRP and PCMH 2011 and PCMH 2014 Recognition Programs are eligible to receive Part IV Maintenance of Certification credit for achieving NCQA Recognition. Individuals must attest to meaningful participation in quality improvement (QI) projects as part of obtaining NCQA recognition. Visit the ABFM website for additional details.

American Board of Internal Medicine (ABIM)
ABIM-certified physicians enrolled in NCQA's Diabetes or Heart/Stroke Recognition Programs can use data from the programs to complete the ABIM Self-Directed PIM and earn Maintenance of Certification credit. Visit the ABIM website for additional details.

The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.

NCQA Patient-Centered Medical Home (PCMH) Recognition is the most widely-used way to transform primary care practices into medical homes.
