CCE Quarterly: Getting PCMH Report Ready for the New Year

The end of the year is the time to start thinking about PCMH reporting for the coming year. Whether you’re working toward PCMH transformation or sustaining your PCMH through annual reporting, knowing how to accurately collect and report patient data is critical to quality measurement and continuous improvement. During this webinar, faculty will discuss how to select populations, reporting periods, timeframes, numerators, denominators and more. This webinar was recorded on December 3, 2019.

Faculty

Amy Tennant, PCMH CCE

Amy Tennant is a Policy Manager in NCQA’s Recognition Programs Policy and Resources. In this role, she creates and maintains PCMH resources, manages product updates and provides policy support and expertise to customers, PCMH CCEs and NCQA Evaluators.

She started at NCQA in the Product Development department where she supported the development of recognition programs including the redesign of the PCMH standards for 2017, Patient-Centered Specialty Practice School-Based Medical Home and the creation of the Oncology Medical Home, the first recognition program created for a specific medical specialty.

Before starting her career at NCQA, she was an intern at MedStar Georgetown University Hospital where she worked on reducing medication reconciliation errors and incidence of outpatient falls. She obtained her Six Sigma Black Belt and holds a BS in Healthcare Management and Policy from Georgetown University in Washington, DC.

 

Amanda Ciadella, MPH, PCMH CCE

Amanda joined The Verden Group in 2017 as Senior Consultant, with a focus on the Florida Chapter of the American Academy of Pediatrics statewide PCMH project where she has supported over 27 practices with their PCMH goals. Her combined background in health administration and her recent work in population health management bring a wealth of knowledge and expertise to her clients. Amanda's education in health care began with a bachelor's degree in health administration and a master's of public health from the University of West Florida. She was also Office Manager in a pediatrics practice in Crystal River, Florida, where she participated in the Blue Cross Blue Shield Florida pilot PCMH program and helped the practice earn NCQA Recognition. Dedicated to expanding her knowledge of PCMH and helping clients navigate the path to NCQA Recognition, Amanda has been a PCMH CCE since 2014 and has supported practices under the PCMH 2011, 2014 and 2017 standards. Amanda was a 2018 – 2019 PCMH CCE Quality Award winner.

Continuing Education

This webinar recording provides maintenance of certification credit (2 points) under "required continuing education" for PCMH CCEs.  This is a non AMA PRA Category 1 CreditTM, ANA CNE, APA, ACPE and ASWB ACE activity.
