Improving Care for Chronic Kidney Disease: Learning From Patients, Doctors and Health Systems

This course will highlight recently-developed tools and resources related to enhancing the quality of chronic kidney disease (CKD) care including the Office of Minority Health and NCQA’s CKD Disparities: Educational Guide for Primary Care, Bayer and NCQA’s Kidney Health Toolkit, and a new performance measure on kidney health evaluations for people with diabetes. The course will also feature a panel discussion with input from patient, health plan, and provider perspectives on how to improve CKD care and outcomes..

Shannon Davis, MSN, APRN, FNP-C, RCEP, CDCES
Senior Manager Quality Improvement

Growing up in a small town in Western Oklahoma, Shannon Davis had a big interest in healthcare. Although she didn’t know exactly where she would land, she wanted to explore the healthcare industry. After attending Oklahoma State University and earning a B.S. in Nutritional Sciences, she knew her journey to pursue an education in healthcare was just beginning. She immediately went on to earn an M.S. in Applied Exercise Science and after starting her career as a Registered Clinical Exercise Physiologist she went on to become a Registered Nurse and later completed practical requirements to become a Certified Diabetes Care and Education Specialist. Her journey didn’t stop there as she went on to pursue her MSN and become a Family Nurse Practitioner. She currently works in Quality Improvement at Health Care Service Corporation (HCSC). During her time at HCSC she has developed and implemented processes around various quality measures that have led to year over year performance improvement. Prior to her time at HCSC, she worked in various hospital settings with a predominant focus on cardiopulmonary and diabetes health. As a professional she has had a longstanding passion for improving healthcare outcomes, quality of care, and quality of life for the patients and members she has served.


Dawn P. Edwards
Patient Advocate

Self-described 27 Year Chronic Kidney Disease Warrior, Dawn P. Edwards has experienced first-hand every renal replacement modality including a kidney transplant and rejection. She is currently a Nocturnal Home Hemodialysis patient. As a result, she has a plethora of insight into the needs of people with Chronic Kidney Disease, always keeping in mind that patients need different things at different times in their lives. Dawn is dedicated to improving the quality of life of people with kidney disease, and is also a resource to her community, sharing her story and educating people about the relationship between hypertension, diabetes and kidney disease, especially among underserved communities.

Dawn has served the community for over 25 years through the IPRO ESRD Network of New York as a communicator, mentor and educator, she serves on many kidney disease related boards and has reinvented herself by working as a Patient Advocate for Fresenius/NxStage and a Wellness Ambassador for the Rogosin Institute. Dawn is also the Co- Chairperson of the Forum of ESRD Networks Patient Advisory Council (KPAC), is actively involved with the National Kidney Foundation and American Association of Kidney Patients, serves as Patient Advisor for Studies with the NIH/NIDDK, serves on the NIH-NIDDK Advisory Council, and is the CEO of her own Advocacy and Education Organization, the New York State CKD Champions, whose motto is to Educate, Encourage and Empower. She believes that Faith Family and Education are the keys to her longevity, and every day is an opportunity to thrive, not just survive.

Dawn recently co-authored an article in the January 2021 issue of Clinical Journal of the American Society of Nephrology (CJASN) titled, Personal Experiences of Patients in the Interaction of Culture and Kidney Disease.


Alyssa Hart, MPH
Senior Healthcare Analyst

Alyssa M. Hart, MPH, is a Manager in the Quality Solutions Group at NCQA. Ms. Hart provides both analytical and technical assistance for federal contracts focusing on quality measurement and improvement. She currently works on the development and maintenance of the quality measurement methodology, measure specifications and benchmarks for the End-Stage Renal Disease (ESRD) Care (CEC) Model and ESRD Treatment Choices (ETC) Model, both quality improvement programs aimed at improving ESRD patient outcomes and provider performance. She also works with CMS’ Office of Minority Health to identify and address the causes of disparities in chronic kidney disease and ESRD. Ms. Hart also provides technical assistance and data analytics for the Centers for Medicaid and CHIP Services (CMCS) measures included in the Adult, Child, and Health Home Core Sets.

Prior to joining NCQA, Ms. Hart was a research assistant and graduate teaching assistant at the George Washington University’s Milken Institute School of Public Health where she conducted research on the implementation of the International Health Regulations, performance measurement within the hospice sector, and veteran health policy. Ms. Hart has a Master’s degree in in Public Health Policy from the George Washington University.


Emily Hubbard
Health Care Analyst

Emily Hubbard is a Health Care Analyst in the Performance Measurement Department at the National Committee for Quality Assurance (NCQA).

Emily leads the development and maintenance of performance measures focused on diabetes and kidney care. She also manages multi-stakeholder groups, including the Diabetes Measurement Advisory Panel and the Care Coordination Work Group.

Emily holds a Bachelor of Science in Anthropology and a Bachelor of Science in Public & Nonprofit Administration, with a specialty in Community Health, from Grand Valley State University.


Karen Onstad, MPH, MPP
Director, Quality Solutions Group

Karen Onstad, MPH is the Director of Quality Solutions Group at NCQA. Ms. Onsted has over 20 years of experience leading quality measurement initiatives, both nationally and regionally. NCQA initiatives implemented under Karen’s leadership include: CMMI Comprehensive End Stage Renal Disease Care Model, CMS Medicaid Core Set Measures, CMS Health Equity Innovation Incubator, and the Delaware State Innovation Model. Karen holds of Master of Public Health from the University of California, Berkeley.


Delphine S. Tuot, MDCM, MAS
Associate Professor of Medicine

Delphine S. Tuot, MDCM, MAS, is an Associate Professor of Medicine in UCSF’s Division of Nephrology at Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG). She also serves as the Associate Chief Medical Officer for Ambulatory Specialty Care and Diagnostics at ZSFG and Director of the UCSF Center for Innovation in Access and Quality. Her current work focuses on the development, implementation and evaluation of interventions that increase patient engagement in health, translate evidence into practice for patients with kidney disease, and enhance access to specialty care for underserved populations.
