Getting Recognized

Practices that earn NCQA PCMH Recognition demonstrate an ongoing commitment to patient-centered care and quality improvement. The PCMH standards and guidelines document contains PCMH Recognition program requirements and information your practice needs to demonstrate to NCQA that you meet criteria. 

Practices transform their practice over time, building on successes. Along the way, NCQA conducts check-ins with the practice to gauge progress and to discuss next steps in evaluating that progress. Practices that meet or exceed PCMH criteria earn the NCQA seal, demonstrating their commitment to strong, patient-centered care.
