NCQA Announces New Resource Guide for Health Care Organizations

Population Health Management Resource Guide will help health plans achieve population health goals

WASHINGTON, DC—The National Committee for Quality Assurance (NCQA) is pleased to announce the release of thePopulation Health Management Resource Guide, a tool to support health plans in developing and identifying best practices for achieving their population health management goals.

NCQA, the industry expert in quality, developed this guide in collaboration with Janssen Scientific Affairs, LLC (Janssen) for health plans implementing the new population health management (PHM) category for 2018 Health Plan Accreditation. The guide walks through five components of creating a PHM strategy and includes explanations, definitions, FAQs and relevant NCQA programs and measures. The guide also includes in-the-field examples of population health management in action from 15 organizations across the country.
