Following recent reports from the World Health Organization (WHO) and the Centers for Disease Control and Prevention (CDC), NCQA has decided to cancel our seminar offerings in May in FL. 

Gov. Ron DeSantis directed State Health Officer and Surgeon General Dr. Scott Rivkees to declare a public health emergency in Florida on March 1, 2020.  Our top priority is the health and safety of our attendees.

The 6th annual Patient-Centered Medical Home (PCMH) Congress helps practices and administrators implement, refine, evaluate, and measure the medical home approach to patient-centered care.   Gain practical tools, best practices, and insight into value-based care, evolving health IT trends, and emerging quality data. Learn successful strategies for achieving and sustaining PCMH recognition for medical homes from any care setting. 

 


PCMH Recognition Annual Reporting: Continued Success

This education program is designed to help PCMH practices understand the larger goals of Annual Reporting and prepare for annual submission under the Patient-Centered Medical Home program. Expectations for those transitioning from the 2014 PCMH recognition as well as those that have achieved Recognition under the current PCMH requirements will be discussed in detail.

Faculty will highlight ways to demonstrate sustained recognition status through maintenance and continuous quality improvement efforts using data that the practice is already collecting internally and for other sources (e.g. state initiatives). Timelines for deliverables and performance expectations will be key discussion items.

  • Outline the Annual Reporting Requirements for PCMH recognition.
  • Explain how Annual Reporting supports the maintenance of the medical home model and a culture of continuous quality improvement within a PCMH practice.
  • Describe the Annual Reporting process, including timelines, submission and audit.
  • Outline important features and functions of QPASS used to submit the Annual Reporting.
