 has decided to cancel our seminar offerings in May in 
 FL. \nGov. Ron DeSantis directed State Health Officer and 
 Surgeon General Dr. Scott Rivkees to declare a public 
 health emergency in Florida on March 1\, 2020.  Our top

 Patient-Centered Medical Home (PCMH) Congress helps

 of Annual Reporting and prepare for annual submission under 
 the Patient-Centered Medical Home program. Expectations for 
 those transitioning from the 2014 PCMH recognition as well 
 as those that have achieved Recognition under the current

 highlight ways to demonstrate sustained recognition status 
 through maintenance and continuous quality improvement 
 efforts using data that the practice is already collecting 
 internally and for other sources (e.g. state initiatives).
