Faculty

Sarah Lee, PCMH CCE

Sarah Lee has been at NCQA since 2012 and is the Assistant Director, Recognition Programs, Policy and Resources. She supports the development, maintenance and update of NCQA's practice-level recognition and state-specific certification programs. She als focuses on providing both internal staff and external customers with the resources needed to clearly understand recognition program processes and requirements. Prior to this role, she was a manager of Government Recognition Initiatives where she oversaw the review process for federal qualified health centers and military facilities pursuing NCQA recognition.

Sarah graduated from the University of California San Diego with a B.A. in International Studies, Economics. She received her M.P.A with a focus on Policy Analysis and Evaluation from The George Washington University in Washington, DC.

Jennifer Martin, MHA, PCMH CCE

Jennifer Martin is the CEO of Quality HealthCare Consulting, LLC and an NCQA PCMH Certified Content Expert (CCE) with over thirteen years of experience.  She a forward-thinking, innovative consultant passionate about improving quality care and committed to leading change. Martin has reviewed over 180 PCMH applications. Her clients range from small practices to large integrated delivery networks. Of the groups that have submitted their NCQA PCMH applications under her direction through 2018 (170 practice-sites), 93% achieved Level 3 Recognition. Martin has spoken nationally and has served as a mentor to individuals: pursuing CCE recognition, quality directors of large Integrated Delivery Networks, and pharmaceutical representatives. Martin spent her career building relationships with physicians that have helped her facilitate practice transformation. Prior to becoming a full-time consultant, Martin gained extensive experience as the Director of Quality and Outcomes at Regional HealthPlus (RHP), an Accountable Care Organization (ACO) affiliated with Spartanburg Regional Healthcare System. She was responsible for developing strategies to measure, align, and deliver high-quality care to patients. She has designed clinically and financially integrated physician engagement models that have enabled physicians to receive reimbursement for achieving meaningful outcomes. She developed strategies to position RHP to meet and exceed Medicare Shared Savings Program (MSSP Track 1) and Quality Payment Program (QPP) criteria. Further, she created risk-adjustment strategies using Hierarchical Condition Category (HCC) coding to maximize gain share arrangements. Prior to joining RHP, Martin directed half of the corporate office at Palmetto Primary Care Physicians (PPCP)—a group of 100+ providers in Charleston, SC. There, she was responsible for leading quality initiatives, including South Carolina’s first Patient Centered Medical Home contract with BlueCross BlueShield (2008).

 Daniel McGill, MA, PCMH CCE                                     

Daniel McGill is a Program Manager with NCQA’s Recognition Operations team. He has spent the last six years working with a variety of private practices and federally qualified health centers as part of NCQA’s ongoing efforts to assist and guide healthcare providers in their transformation into highly efficient and effective patient centered medical homes. Prior to joining NCQA, Daniel worked in various membership development and program management capacities at the National Patient Safety Foundation in Boston, MA and the American Physical Therapy Association in Alexandria, VA. In addition to his PCMH CEC, he has received a master’s degree in Public Communication from American University in Washington, DC and a bachelor’s degree in Communication from James Madison University in Harrisonburg, VA.
