Piedmont Health is a health center supported by the Health Resources & Services Administration (HRSA). This HRSA-commissioned video demonstrates Piedmont’s success with the Patient-Centered Medical Home concept of care and details all the places the model can make a difference–for patients, staff, and even the bottom line. Piedmont encourages all providers, and health centers in particular, to pursue PCMH Recognition.
Piedmont Health serves nearly 50,000 people, including 26,000 uninsured patients. Providing the high quality, affordable health care their patients need starts with a team-based, patient-centered approach. Piedmont reports that Earning PCMH Recognition improved clinical outcomes, staff satisfaction, and helped patients like Craig Redding live healthier lives.
