Rising Cut Points Demand Smarter Home-Based Gap Closure

As Star Ratings cut points continue to rise, health plans face increasing pressure to close more care gaps, particularly across high-impact measures like GSD, KED and CCS. While healthcare organizations have unprecedented data at their disposal, bridging the final gap in care delivery remains a persistent hurdle. The core challenge extends beyond poor engagement—it lies in recognizing that different care gaps and member needs demand tailored approaches. Broad strategies like blast mailers, uniform testing kits, or limited home visits fail to maximize return on investment. Instead, health plans need a more precise approach that first determines whether a gap is truly open, then matches each member to the right intervention and care modality. This webinar brings together two home-based care partners, Ash and Sprinter Health, who have combined their respective in-home care models to deliver care that meets members when and how it’s needed. Together with a mutual health plan partner, they’ve built a joint program that uses one shared set of member data to decide, gap by gap, what action is needed and which modality is most likely to close it, from existing clinical data to in-person and/or virtual care to at-home testing — and at what cost.
