A Look Inside the 6 PCMH Concepts: A Comprehensive Overview

Intermediate level sealNCQA’s PCMH Recognition program has evolved to feature a set of six concepts that make up a medical home. Underlying these concepts are criteria (activities for which a practice must demonstrate adequate performance to obtain NCQA PCMH Recognition) developed from evidence-based guidelines and best practices.

Each concept’s criteria outline the features of PCMH transformation and how NCQA evaluates a practice’s ability to function as a patient-centered medical home. You will review an overview and description for each standard, as well as criteria examples, to help you in the transformation process.

This self-paced training focuses on the six PCMH concepts listed below:

  1. Team-Based Care and Practice Organization (TC)
  2. Knowing and Managing Your Patients (KM)
  3. Patient-Centered Access and Continuity (AC)
  4. Care Management and Support (CM)
  5. Care Coordination and Care Transitions (CC)
  6. Performance Measurement and Quality Improvement (QI)

What You Will Learn

At the conclusion of the module, participants will have the knowledge they need to:

  1. Discuss the PCMH requirements, the virtual check-in process and the transformation journey.
  2. Outline core and elective criteria for the 6 concepts.
  3. Explain criteria scoring in the standards and guidelines. 
  4. Discuss examples that meet core criteria and electives.
