About this Webinar 

Behavioral health problems frequently present first to the primary care setting and have significant effects on physical health as well as on language and communication skills, cognitive development, social and emotional development.  For the provider and health care team, addressing behavioral health issues can be difficult to establish in a timely and resourceful manner. Despite this challenge, pediatric practices are uniquely poised to take an innovative approach to support both the patient, family and the health care team by integrating behavioral health care into pediatric primary care practices. Practice transformation uses data to support the need for additional resources, define and change existing job descriptions and adding additional staff. Identifying and implementing processes, tools, warm hand offs and building relationships with community resources has created a successful behavioral health care integration, closed loop tracking and improved patient outcomes  in primary care pediatric practices.

What Will You Learn

At the conclusion of the module, participants will have the knowledge they need to:

• Examine ways to assess current practice for data collection to determine what resources are needed and who should be part of your behavioral health care team.

• Identify screenings, tools and assessments as well as opportunities for behavioral health care touch points and outreach.

• Discuss strategies for building relationships with community resources, warm hand offs and closing the loop.
