About the Speakers

Mina L. Harkins, MBA, BSMT, PCMH CCE

Mina L. Harkins was with the National Committee for Quality Assurance (NCQA) for 10 years as the Assistant Vice President, Recognition Programs Policy and Resources, in Washington DC. In that position, she was responsible for the content of and resources to support all Recognition Programs. The Recognition programs are focused on evaluating quality management of chronic conditions, such as Diabetes and Ischemic Vascular Disease, and the systems and processes utilized in clinical practices including the Patient-Centered Medical Home, the Patient-Centered Specialty Practice program, Patient-Centered Connected Care in the neighborhood and Accountable Care Organization Accreditation. Before joining NCQA, she directed a medical laboratory accreditation program, administered a large anatomic pathology operation, managed several units in a multi-specialty group practice and a multi-site laboratory operation for a staff model HMO. Ms. Harkins earned her BA in Medical Technology at the University of Pittsburgh and her MBA at Johns Hopkins University’s Carey School of Business.

Angel E. Perez, BSN, PCMH CCE

Angel Perez is a Practice Transformation Coach who partners with Primary Care Clinics helping practices transform to deliver patient-centered care. Emphasized areas include training, support, encouragement, consultation, and monitoring to both practices and health systems as they strive for clinical and operational improvements.  Angel has a background in nursing and has over 20 years of experience in the healthcare in several areas including Public Health, Women’s Health, Ortho Surgery, Tissue recover and quality improvement. More recent years sharpening her skills in development and implementation of learning collaboratives focused on supporting practices and health systems in transforming the way they deliver care. Angel holds a BSN and is an NCQA Certified Content Expert.
