About the On-Demand Training

These online modules are designed for organizations preparing for the new Accreditation of Case Management for Long-Term Services and Supports (CM-LTSS). Expert faculty will provide an in-depth review of the CM-LTSS standards. Participants will learn about the survey process, program eligibility, how requirements align with Health Plan Accreditation and how to apply the standards across a variety of settings such as Accountable Care Organizations (ACOs), Patient-Centered Medical Homes (PCMHs) as well as home and community-based organizations (e.g. area agencies on aging).

Faculty

Toshira Moffet, MPH 

Torshira Moffett is Assistant Director of Product Development at the National Committee for Quality Assurance (NCQA). Torshira researches opportunities for NCQA to develop new evaluation programs. In collaboration with internal staff and external stakeholders, she manages the development of standards and guidelines for new programs and updates to existing programs to align with current industry practices. One of her most recent projects includes updating NCQA’s Case Management Accreditation to better address populations with complex care needs. In addition to her work in Product Development, she is serving as a navigator for organizations participating in NCQA’s Learning Collaborative to pilot new standards for long-term services and supports to include in Case Management Accreditation.

Prior to joining NCQA, Torshira interned at the National Governors Association Center for Best Practices in the Health Division. She also worked as a diabetes program developer at Rochester Primary Care Network in Rochester, NY.

Torshira holds a Master of Public Health in Health Policy from the George Washington University. She also received a Bachelor of Arts in Sociology and Political Science from the University of North Carolina at Chapel Hill.

Teresa M. Treiger, RN-BC, MA, CHCQM-CM/TOC, CCM

Teresa Treiger is a thought leader inspiring Conscious Case Management© practice across the health care continuum. With over thirty years of health care industry experience, Teresa is an expert in professional case management practice. She is principal at Ascent Care Management, LLC providing private case management, consultative guidance, and educational programs to diverse organizations across the health care continuum. Prior to establishing ACM, Teresa worked at various health care organizations including McKesson Corporation, Blue Cross and Blue Shield of Massachusetts, and Harvard Pilgrim Health Care.

Teresa is a prolific author in journals and white papers as well as a chapter contributor to academic texts, most recently the Case and Population Health Management chapter in Leadership and Nursing Case Management, 6th ed. (with E. Fink-Samnick). Teresa co-authored COLLABORATE© for Professional Case Management: A Universal Competency Based Paradigm (with E. Fink-Samnick) and is co-editor and contributing author of the CMSA Core Curriculum for Case Management, 3rd ed. (with H. Tahan). She serves as a certification examination item writer and reviewer for both the Commission for Case Management Certification and the American Board of Quality

In support of improving patient care, the National Committee for Quality Assurance (NCQA) is accredited by the American Nurses Credentialing Center (ANCC), the Accreditation Council for Pharmacy Education (ACPE), and the Accreditation Council for Continuing Medical Education (ACCME), to provide continuing education for the healthcare team.
