Continuous QI & DEI – Better Knowing and Managing Patients 

How much we know about our patients determines the quality of care we provide. Social determinants of health (SDOH) and other factors make up 70% of patient’s overall outcomes. Patient Centered Medical Home (PCMH)- Knowing and Managing Patients concept can be used as a foundation to incorporate SDOH, diversity, equity, and inclusion components in with practice workflows, and EMRs to drive reports and offer focus areas for quality improvement.  

 During this session we will use PCMH as a foundation to introduce diversity, equity, and inclusion through knowing and managing patients, and continuous quality improvement. 

What You Will Learn

After successful completion of this activity, learners will be able to: 

  • Realize criteria in PCMH Knowing and Managing Patients.  
  • Utilize a comprehensive health assessment to gather SDOH. 
  • Identify how Knowing and Managing patients can impact whole person care and drive positive outcomes related to health equity.  
  • Leverage the EMR for workflows, and data sharing for continuous QI and education.

Kelsey Keith, BSPH, PCMH CEC 

Kelsey Keith is the Patient Centered Medical Home Program Manager for Northeast Georgia Physician’s Group (NGPG). In her role she manages all NCQA programs for NGPG including Patient Centered Medical Home (PCMH), Diabetes Recognition Program (DRP), and Patient Centered Connected Care (PCCC). In addition to managing NCQA programs Kelsey champions Patient Experience across the ambulatory care clinics and is responsible for directing the Patient Advisory Groups each month. She has nine years of experience in practice transformation and has led and sustained more than sixty ambulatory clinics in achieving Patient Centered Medical Home, twenty-six clinics achieving DRP, and seven Urgent Care Clinics maintaining PCCC. Kelsey has a Bachelor of Science in Public Health from East Carolina University and has been a CCE since July of 2014.

Jennifer Anglin, MS, CHES, PCMH CCE

Jennifer Anglin is a Manager of Recognition Policy at NCQA. Her responsibilities include creation and maintenance of PCMH resources and providing support to practices going through transformation or annual review. Prior to NCQA, Jennifer worked for Purdue University by assisting practices throughout the state of Indiana on quality improvement and research projects as well as supporting them to meet Medicaid’s Promoting Interoperability program and Medicare’s MIPS program. Jennifer’s professional interests include health policy,, research, and education. She has a particular interest in social determinants of health and the behavioral health aspect of Patient-Centered Medical Homes.

Jennifer holds a BA in secondary education from Valparaiso University and a master’s degree in health science from Indiana State University. Jennifer is a Certified Health Education Specialist and has been an NCQA Patient-Centered Medical Home Certified Content Expert (PCMH CCE) since 2013.

In support of improving patient care, the National Committee for Quality Assurance is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC) to provide Interprofessional Continuing Education for the healthcare team.  

This activity was planned by and for the healthcare team, and learners will receive 1.0 Interprofessional Continuing Education (IPCE) credit for learning and change.  
 
This educational activity is approved for a maximum of 1.0 AMA PRA Category 1 Credit™, and 1.0 nursing contact hours. 

This live course grants 2.0 required Continuing Education Unit (CEU) points for PCMH Certified Content Experts.
