Harnessing the Power of Diagnostic Data to Support Care Management, Quality Improvement and Cost Reductions

Laboratory data can be a powerful tool for understanding the health of an organization’s patient population and for guiding a patient’s diagnosis and treatment. Lab data and analytics can turn into actionable insights for physicians, providers, administrative leaders and care management team members, and can help support overall improvement and standardization in health care delivery.

Did you know that laboratory testing is one of the highest-volume medical activities? Laboratory data inform about 70% of clinical decisions—and testing is also used to screen, diagnose and monitor many of the 131 HCC codes defined by CMS. Integrating laboratory services into management of population health gives you a closer look at the patients you serve.  

This session discusses the often-overlooked availability and use of lab data to target and support improvements in health care delivery, and reduce costs, while optimizing performance in the PCMH model of care.

What You Will Learn

At the conclusion of this program, participants will be able to:

•    List the significant role laboratory data play in clinical decisions, population health management, identification of rising risk/higher risk patients and reduction in overall health care costs.
•    Describe how diagnostic laboratory data can support actions across stakeholder groups, including physicians/providers, administrators, care managements teams and patients, to guide a culture of data-driven performance improvement to enhance quality and reduce costs (Performance Measurement and Quality Improvement; QI 01, QI 02).
•    Demonstrate how the use of interactive lab data dashboards can help practices identify patient needs at the individual and population levels to categorize those who could benefit from care management, due to poorly controlled or complex conditions, and those at highest risk of poor health outcomes (Care Management and Support; CM 01, CM 02, CM 03).
 

Who Should Attend

This webinar is geared towards individuals with one year of experience, or more; collecting, reporting, and acting upon annual reporting data; or those who need a refresher. This program is designed for individuals in the practice that work with patients, and with PCMH recognition.
