Preventing Diabetic Complications: An FQHC Approach

Swope Health Services serves a vulnerable underserved patient population that has many high-risk factors for complications of diabetes. Using PCMH principles, it built an EMR (e clinical works) tool for effective previsit planning—even in an open access setting—and linked it to other electronic tools, such as in-office retinal exam devices, to improve diabetes outcomes and focus on tertiary prevention.

Faculty

Naiomi Jamal, MD

Naiomi Jamal, MD, is Chief Quality Officer at Swope Health, a Federally Qualified Health Center. She heads the department of Quality and Population Health and is responsible for developing the organization’s annual Quality Plan and detailed Clinical Quality Improvement Summary. She is lead physician for the PCMH, provides primary care services and oversees organizationwide clinical quality and performance improvement, along with population health initiatives centered around the quadruple aim of improving the provider and patient experience, improving population health outcomes and resource stewardship.

Dr. Jamal also serves as faculty at the University of Missouri Kansas City, developing and delivering a preventive medicine and public health curriculum for family medicine residents. She also developed the didactic lecture series, “Social and Structural Determinants of Health” at the UMKC school of medicine.

Dr. Jamal graduated from the Family Medicine residency at the University of Texas Medical Branch, and completed a second residency in General Preventive Medicine and Public Health. She is board certified in Medical Quality.
