A Look Inside PCMH Concept: Performance Measurement & Quality Improvement (QI) 

NCQA’s PCMH Recognition program has evolved to feature a set of six concepts that make up a medical home. Underlying these concepts are criteria (activities for which a practice must demonstrate adequate performance to obtain NCQA PCMH Recognition) developed from evidence-based guidelines and best practices.

This self-paced module focuses on Performance Measurement & Quality Improvement (QI). The QI concept focuses on measuring performance to assess how processes put into place support the Patient-Centered Medical Home. It ensures processes are working to enhance quality, reduce costs, and improve patient experience. Measurement enables practices to have a concrete way to see how they're doing and act upon that information, if needed. Without measurement, a practice wouldn't be able to gauge if processes are working effectively and efficiently. A vital piece of being a PCMH is ensuring that there are goals and assessments in place for continuous quality improvement.

What You Will Learn

At the conclusion of the module, participants will have the knowledge they need to:

  1. Discuss the format of PCMH requirements, the virtual check-in process and the transformation journey.
  2. Outline core and elective criteria for Performance Measurement and Quality Improvement (QI).
  3. Explain how QI is scored using scoring criteria within the standards and guidelines.
  4. Discuss examples that meet core criteria and electives.
