Application Reminder

Remember that your organization’s application, signed contract and application fee are due nine months before the start date of your survey. You can order the HPA Application Packet here. If you have questions about the application materials, contact your NCQA Applications and Scheduling Account Representative (ASAR). If you don’t know the name of your ASAR, e-mail us at to request contact information.

Policy Clarification Support

If you have questions about the standards and guidelines, you can submit them to NCQA through our online Policy Clarification Support (PCS) system and receive written answers from our Policy team.
