Tables Usage Agreement
Use of the National Committee for Quality Assurance’s (“NCQA”) Risk Adjustment Tables (the “Tables”) constitutes acceptance of our terms of use as set forth in this usage agreement (the “Agreement”).
The Tables were developed by and are owned by NCQA. The Tables may only be used, without modification, for a non-commercial purpose to assist with calculation of NCQA’s Risk Adjusted Utilization HEDIS® measures. NCQA holds a copyright in these materials and can rescind or alter these materials at any time. You may not use the Tables for any other purpose, including, but not limited to, a commercial (including but not limited to vendors using the Tables as part of a product or service to calculate HEDIS measure rates) or research purpose, unless you obtain prior written approval from NCQA and agree to the terms of a separate license, at NCQA’s sole discretion.
You may not copy electronically or otherwise transmit, redistribute, republish, modify, adapt, translate, reverse engineer, decompile, disassemble, sublicense or otherwise provide the Tables to any third party. You may not use or include the Tables in any other print or electronic publication, service or product.
This Agreement shall become effective immediately upon your access of the Tables.
YOU CONFIRM YOUR AGREEMENT TO VOLUNTARILY USE THE TABLES SUBJECT TO THE TERMS AND CONDITIONS OF THIS AGREEMENT BY USING THE TABLES FOR HEDIS MEASURE CALCULATION.
IF YOU DO NOT AGREE TO THE TERMS STATED ABOVE YOU ARE PROHIBITED FROM USING ANY OR ALL OF THE TABLES.
