Blue Cross Blue Shield of Massachusetts (Blue Cross) shared the results of a new analysis of racial and ethnic inequities in health care and became the first health plan in Massachusetts to announce it will incorporate equity measures.

The OnDemand will also discuss how to implement the person-driven outcomes approach in clinical care, as well as the benefits of the approach for individuals and their care team.

This course will provide primary care clinicians with education to support earlier diagnosis of heart failure, establish a treatment plan and monitor disease progression in order to improve patient outcomes and reduce disease burden.
