Health Equity Accreditation Plus: Helping Improve Care for Your Community

Does your organization have an NCQA Health Equity Accreditation status? Maybe your organization is considering how this program aligns with its goals for quality improvement and population health.

The Health Equity Plus milestone, in the NCQA Health Equity Accreditation program, recognizes health care organizations that have implemented practices to address community social or environmental factors influencing patients’ health outcomes. Program standards are a framework for collecting nonclinical social needs data at the patient and community levels, establishing meaningful community-based partnerships and connecting patients to social needs resources.
