Digital Health Engagement Early Adopters Application

NCQA recently launched its new Accreditation in Digital Health Engagement (DHE), a three-year accreditation designed for digital health solutions, tools, and apps that empower individuals to manage their health and wellbeing between provider visits through assessments, self-management tools, coaching, and other digitally enabled supports. The program includes a set of core standards and two accreditation modules: Health Assessment and Digitally Enabled Interventions. Organizations must complete the core standards and at least one module to achieve accreditation. To help organizations successfully navigate their first NCQA accreditation experience, we are pleased to offer the DHE Early Adopter Cohort. This complimentary cohort is designed for organizations that are new to NCQA Accreditation and are committed to building the quality, measurement, and governance capabilities needed to demonstrate program performance and outcomes. Through a combination of expert guidance, educational resources, structured touchpoints, and peer learning opportunities, participants will receive support throughout their accreditation journey. Once you have completed this form and your eligibility is confirmed, NCQA will provide the link to purchase your survey and information on next steps.

How to Participate: 

  • Step 1: Review the DHE Standards and Readiness Package: Purchase the DHE Standards and Readiness Package, which includes the final version of the standards as well as educational resources designed to help your organization prepare for accreditation success. 
  • Step 2: Submit Your Early Adopter Cohort Application: Complete the application below for NCQA review. Additional information about the cohort can be found here. 
  • Step 3: Purchase Your Accreditation Survey: Organizations selected for the cohort must purchase their accreditation survey to secure their spot. Once your application is reviewed and eligibility is confirmed, NCQA will provide a survey purchase link and information about next steps. Participation in the cohort requires survey purchase by August 31, 2026. 

Key Dates:  

  • August 31, 2026: Deadline to purchase your survey and secure a place in the cohort. 
  • September 4, 2026: Early Adopter Cohort applications due 
  • Week of September 14, 2026: Cohort kickoff and orientation
