About the On-Demand Training

These online modules are designed for organizations preparing for the new Accreditation of Case Management for Long-Term Services and Supports (CM-LTSS). Expert faculty will provide an in-depth review of the CM-LTSS standards. Participants will learn about the survey process, program eligibility, how requirements align with Health Plan Accreditation and how to apply the standards across a variety of settings such as Accountable Care Organizations (ACOs), Patient-Centered Medical Homes (PCMHs) as well as home and community-based organizations (e.g. area agencies on aging).

Faculty

Candice Costello, BS 

Candice Costello joined NCQA in 2010 as a Health Care Analyst in Performance Measurement. In this role, she assisted with the development and management of NCQA’s HEDIS Measure Set, as well as supported numerous research projects focused on children’s’ and women’s health. 

In 2013, Candice joined NCQA’s Accreditation and Recognition Department as an Accreditation Manager. In this role, her primary responsibilities are serving as an Accreditation Survey Coordinator and Administrative Surveyor for organizations seeking NCQA Accreditation and Certification. Although Candice coordinates and surveys a wide variety of the different NCQA survey products, she is the product champion and co-champion for NCQA’s new Case Management for Long-Term Services and Supports (LTSS) Accreditation product and Physician and Hospital Quality (PHQ) product. 

Continuing Education

In support of improving patient care, the National Committee for Quality Assurance (NCQA) is accredited by the American Nurses Credentialing Center (ANCC), the Accreditation Council for Pharmacy Education (ACPE), and the Accreditation Council for Continuing Medical Education (ACCME), to provide continuing education for the healthcare team.
