For Medicaid sanctions, organizations may use any of the following sources:Â
- State Medicaid agency.Â
- AMA Physician Master File.Â
- Federation of State Medical Boards (FSMB).Â
- National Practitioner Data Bank (NPDB).Â
- SAM.gov.Â
For Medicaid exclusions, organizations may use any of the following sources:Â
- State Medicaid agency.Â
- List of Excluded Individuals and Entities (LEIE) maintained by the Office of Inspector General (OIG).Â
- National Practitioner Data Bank (NPDB).Â
