Unmet social needs such as food insecurity, lack of stable or adequate housing and transportation inaccessibility are linked to poorer access to care and worse clinical outcomes. NCQA introduced the Social Need Screening and Intervention (SNS-E) measure for HEDIS® MY 2023 to evaluate how health care organizations identify non-medical drivers of health and take action to help members access needed services and supports.
The measure uses the Electronic Clinical Data Systems reporting method, which leverages data from EHRs, registries, HIEs, case management systems and claims. NCQA updated the measure for MY 2026 to include additional data sources and new billing codes.

To assist health plans in implementing the changes, NCQA released a new resource, Social Needs Screening and Intervention: Measure Updates for HEDIS MY 2026 and Frequently Asked Questions. This document includes an updated measure description, a visual depiction of the measure and frequently asked questions about coding and measure calculations.
