A handful of NCQA HSRP and PCMH-Recognized practices shared their stories of delivering quality cardiovascular care in a series of spotlight documents NCQA wrote as part of the Centers for Disease Control’s Million Hearts campaign.

High performers in cardiovascular care and quality were selected based on their performance on quality measures and their location, organizational structure and patient populations.
