NCQA has a long history of developing products and programs to meet identified needs in health care — It’s how we drive quality.

We are proud to continue that stewardship with a new guide for organizations that assist the most vulnerable among us: the elderly and the disabled.

A Roadmap to Success in LTSS points organizations toward meeting NCQA standards for accreditation or distinction for case management of LTSS – Long-Term Services and Supports. LTSS are health care services provided by organizations to care for people where they choose to live: in their own homes, with loved ones, in their communities…where they are most comfortable and happy.

We can’t do this kind of work alone! Support for the guide came from The SCAN Foundation and The John A. Hartford Foundation and others who also value accountability in health care.

Hartford Foundation to help organizations that provide care to our most vulnerable populations—to ensure that they deliver quality care that coordinates with all of a patient’s health care and supportive service providers.”

A Roadmap to Success contains tools, examples and links to valuable resources to help organizations prove their ability to meet NCQA’s LTSS standards. Our goal is to make the accreditation process easy to navigate and to help organizations interpret and implement the standards.

NCQA developed the Case Management for LTSS Accreditation program and the LTSS Distinction modules as an opportunity for community-based organizations, health plans, case management organizations and managed behavioral healthcare organizations to demonstrate their ability to deliver Long-Term Services and Supports and coordinate with other health care and service providers.
