NCQA invites stakeholders to review and provide feedback on proposed updates to 2027 Patient-Centered Medical Home (PCMH) Recognition and to a new program, Wellness and Condition Management.
What Is Public Comment?
Public comment participants from across the health care ecosystem—including consumers—submit feedback directly to NCQA about proposed updates to our programs.
Public comment is an important part of standards development: It helps ensure that updates are clearly written, feasible and relevant, and that they align with the needs of organizations across the health care landscape.
About Proposed Updates
2027 PCMH Recognition
NCQA proposes updates to criteria in the Care Management concept:
- CM 01: Identifying Patients for Care Management. Revise expectations for how pediatric practices identify patients who might benefit from care management.
- CM 02: Monitoring Patients for Care Management. Revise the required number of care plans reviewed by NCQA to better scale with the size of practices.
- CM 04: Person-Centered Care Plans. Revise the minimum requirements for documentation in care plans.
- CM 07: Patient Barriers to Goals. Retire the criterion and incorporate its expectations into CM 04.

Wellness and Condition Management—2026 Draft Updates
Proposed updates modernize and streamline the current Wellness and Health Promotion program to create a new Wellness and Condition Management program. This modular program includes required core standards and up to two modules:
- Core Standards evaluate organizational transparency, data exchange and integration, privacy and confidentiality, participant rights and responsibilities, population assessment, accessible services and measurement of program outcomes.
- Health Assessment Module evaluates an organization’s ability to conduct comprehensive health assessments that inform interventions.
- Digitally Enabled Engagement Module evaluates an organization’s capability to deliver personalized, interactive support that promotes healthy behaviors and reduces member risk.
The modules share common core standards that emphasize measurement of outcomes and member engagement.
