Options for Renewing Practices
As your practice approaches its renewal date, are you trying to decide what path to take? PCMH 2014 ISS survey tools are no longer available, so if you have not purchased a survey tool, your practice must earn recognition through the 2017 PCMH standards and guidelines, using the Q-PASS system. You can learn more about the redesigned process here.
If you purchased the PCMH 2014 survey tool, you have two options:
1. Renew under the redesigned process: Recognized practices can enroll in the redesigned PCMH recognition program under the PCMH 2017 standards and guidelines. Previously earned PCMH credits will be applied to aspects of the 2017 standards.
Note: Practices should commit to the process 6 months before their current recognition expires.
2. Renew under PCMH 2014: Recognized practices can renew recognition for a 3-year period under PCMH 2014 and move to the newly redesigned process at the end of that period. Practices with Level 2 or 3 recognition are eligible for streamlined renewal. The deadline for renewal under the 2014 standards is September 30, 2017.
