PCSP 2016 Eligible Corporate Elements

Organizations submitting with 3 or more practice sites are eligible to submit using the multi-site recognition process. The organization will submit one survey tool at the corporate level, followed by one survey tool per practice site.

Below is a list of the 20 Elements that can be answered in the PCSP 2016 Corporate Survey Tool. All other elements require a response in your site-specific survey tools with site specific information.

Criteria for choosing Corporate Survey Tool Elements:

  • Response to at least 10 of the 20 Elements below in the Corporate Survey Tool.
  • Attest in the documentation or support text/notes of your Corporate Survey Tool that all of the practice sites applying for Recognition share the same policy, procedure or process.
  • If an Element listed is not met by an electronic system or is not a shared policy, procedure, or process you must respond in the site-specific survey tools with site specific information. Note this in the Element text box if applicable.
  • If you choose to respond to a Corporate Element in the site-specific survey tools, do not answer it in your Corporate Survey Tool. Note this in the Element text box if applicable.

After determining which Elements to select for your Corporate Survey Tool, you will need to select them in the Organization Background section of your Corporate Survey Tool.

  • PCSP 3C: Implement Evidence-Based Reminders for Specialty Care (provide data by specialty if the organization is multi-specialty)*
  • PCSP 3D: Implement Evidence-Based Decision Support (provide data by specialty if the organization is multi-specialty)* 
  • PCSP 4C: Use Electronic Prescribing
  • PCSP 5A: Test Tracking and Follow-Up
  • PCSP 5B: Referral Tracking and Follow-Up
  • PCSP 5C: Coordinate Care Transitions (provide data by specialty if the organization is multi-specialty)* 
  • PCSP 6A: Measure Performance (provide data by site and by specialty if the organization is multi-specialty)**
  • PCSP 6B: Measure Patient/Family Experience (provide data by site)
  • PCSP 6D: Report Performance
  • PCSP 6E: Use Certified EHR Technology

Documentation by specialty for the corporate survey tool:

  • For all elements selected for the Multi-site Corporate Survey Tool, data provided must be from the largest site or aggregate data from all sites.

    Exceptions to this guidance include: Elements 3C, 3D, 5C, which always require data by specialty; Elements 6A, which always requires data by site and by specialty; Element 6B, which always require data by site.

  • *For organizations with two (2) specialties, documentation must be provided for both specialties.

    *For organizations with three (3) or more specialties, documentation must be provided for three (3) specialties including 1) largest specialty, represented by number of patient visits, and 2) behavioral health, if included as a specialty. 

  • **If an organization elects to report Element 6A in the corporate survey tool, it must report three clinical measures for each specialty AND by site location for 6A, Factor 1.

  • Documented processes that are the same for all specialties may be provided once to NCQA.
