  • ".5" = Race/Ethnicity Diversity of Membership 
  • "1” = Process measures (e.g., screenings, visits)
  • “1.5” = Patient experience measures (CAHPS)
  • “3” = Outcome and intermediate outcome measures (e.g., HbA1c Control, Blood pressure control)
