NCQA’s Advanced Primary Care Pilot Program is a unique opportunity for your organization to influence the next generation of primary care . Building on the foundation of NCQA PCMH Recognition, the pilot is designed to accelerate innovation, test scalable models and help organizations meet the needs of diverse communities.
The pilot offers early insight into NCQA’s future program, and allows participants to help shape its development. Organizations eligible to participate include independent practices, medical groups, coordinated networks of practices, health systems, CINs and ACOs whose primary care services are managed through shared policies, procedures and quality systems under a common legal and management structure.
Examples of Eligible Organizations

Example A
Independent Practice
Expand - Scenario: A single, stand-alone primary care practice.
- Accreditation Level: Practice-level APC accreditation.
- Why: The practice is fully accountable for its own care delivery and oversight.

Example B
Practice within a Clinically Integrated Network (CIN)
Expand - Scenario: A primary care practice that is part of a CIN but not owned by a health system.
- Accreditation Level: CIN-level accreditation.
- Why: Oversight and accountability for care coordination and quality standards sit with the CIN.

Practice within a Health System and a Regional ACO
Expand - Scenario: A practice owned by a health system, which participates in an ACO.
- Accreditation Level: ACO-level accreditation.
- Why: The ACO holds the highest level of oversight and accountability for care delivery and performance across participating systems and practices.

Practice within a Health System working with a National Care Aggregator
Expand - Scenario: A practice within a health system that belongs to a national Care Aggregator that is comprised of Regional ACOs.
- Accreditation Level: Regional ACO-level accreditation.
- Why: Accountability for care delivery and performance is managed at the regional level, not the national level.
