NCQA’s Diabetes Recognition Program recognizes clinicians who use evidence-based measures to provide high-quality care to their patients with diabetes. The program is designed to help improve care while enhancing practices’ operational efficiency and market positioning. What’s New in Diabetes Recognition?
Requirements. Transitioned from paper-based to digitally specified measures, with new measures phased in starting in January 2026. Submissions are now annual.
Eligibility. Broadened to include any setting that provides care for patients with diabetes.
Submission. Conducted through Q-PASS and linked with Patient-Centered Medical Home (PCMH) Recognition. Requires a random sample of at least 25 patients per clinician. Submissions are digital (CSV or FHIR®), and demographic data are collected via questionnaire.
Why the NCQA Diabetes Recognition Program?
Improve outcomes. Helps implement a framework to strengthen clinical accountability, provides insights to improve.
Prioritize condition management. Focuses on preventing disease progression and addressing related concerns.
Differentiate in the marketplace. Recognized clinicians use evidence-based measures to provide high-quality care to patients with diabetes. Recognized practices and clinicians are listed in NCQA Report Cards.
Succeed in value-based arrangements. Helps provide intermediate outcomes to measure and manage the population while addressing the full spectrum of the diabetes population.
Achieve Medical Board Maintenance of Certification Credits. Clinicians in NCQA-Recognized practices may be able to receive MOC credits toward board-level certifications. Find out if your board offers credit.
Apply to the Program at No Cost
Thanks to grant funding from the Helmsley Foundation, NCQA will cover Diabetes Recognition Program application fees for up to five clinicians at your organization.
Interested organizations with more than five clinicians should complete this form, and an NCQA representative will be in touch to help you through the application process.

To earn NCQA Recognition, clinicians submit data demonstrating that they meet specific criteria for the clinical performance measures the Diabetes Recognition Program comprises.
