About the Program
Behavioral health conditions (mental illnesses and substance use disorders) can often first be identified in a primary care setting, and can be exacerbated by under- or delayed diagnosis and treatment. This is a serious public health problem nationally and across Massachusetts, and there is growing consensus that integration of behavioral health care into primary care is a priority.
This 90-minute webinar will introduce participants to the Health Policy Commission’s recently updated PCMH PRIME Certification Program. NCQA faculty will provide an overview of the PCMH PRIME criteria, documentation requirements, and application process focusing on program updates effective mid-November 2017 (please visit the HPC website for further information about recent PCMH PRIME program changes). The goal of this session is to increase participants’ understanding of how an organization can apply to be PCMH PRIME certified. It will also examine the impact of a behavioral health focus on the medical home model.
This educational webinar session will be presented by William Tulloch, Vicki Gavaghan and Rebecca Best, with a live question-and-answer session following the presentation.

- Explain the criteria and documentation requirements for PCMH PRIME Certification;
- Describe processes and procedures that demonstrate the transformation of a practice towards the PCMH PRIME model;
- Discuss expectations for each requirement; and
- Identify the pathway to becoming PCMH PRIME certified.

This will increase participants’ understanding of how an organization can apply to be PCMH PRIME Certified and the impact of a behavioral health focus on the medical home model.
