Pricing: Multiple Sites 

Multi-site pricing is right for practices that:

  • Have three or more practice sites operating under the same legal entity.
  • Share an EHR system.
  • Have at least some of the same policies and procedures.

Practices seeking recognition for the first time pay the recognition fee at the time of enrollment. Thereafter, they pay the recognition fee at the time of their annual check-in.

Fees apply to each clinician in each practice that earns recognition. 

In the following example, the organization’s initial recognition fee is as follows. 

Practice Site 1: 14 Clinicians 

  • $200/clinician for the first 12 clinicians. 
  • $20/clinician for the next 2 clinicians. 
  • $2,440 subtotal. 

Practice Site 2: 6 Clinicians 

  • $200/clinician for 6 clinicians. 
  • $1,200 subtotal. 

Practice Site 3: 16 Clinicians 

  • $200/clinician for the first 12 clinicians. 
  • $20/clinician for the next 4 clinicians. 
  • $2,480 subtotal. 

Total cost: $6,120 (a $1,530 discount).

Fees apply to each clinician in each practice that earns recognition. 

In the following example, the organization’s initial recognition fee is as follows. 

Practice Site 1: 14 Clinicians 

  • $250/clinician for the first 12 clinicians. 
  • $25/clinician for the next 2 clinicians. 
  • $3,050 subtotal. 

Practice Site 2: 6 Clinicians 

  • $250/clinician for 6 clinicians. 
  • $1,500 subtotal. 

Practice Site 3: 16 Clinicians

  • $250/clinician for the first 12 clinicians. 
  • $25/clinician for the next 4 clinicians. 
  • $3,100 subtotal. 

Total cost: $7,650.

The tiered price approach applies to the recognition fees multi-site practices pay at the time of annual check-in. 

In the example below, the multi-site practice has already earned PCMH 2014 Level 3 status. Moving to the annual process, its fees are as follows. 

Practice Site 1: 14 Clinicians 

  • $120/clinician for the first 12 clinicians. 
  • $12/clinician for the next 2 clinicians. 
  • $1,464 subtotal. 

Practice Site 2: 6 Clinicians 

  • $120/clinician for six clinicians, 
  • $720 sub-total. 

Practice Site 3: 16 Clinicians 

  • $120/clinician for the first 12 clinicians. 
  • $12/clinician for the next 4 clinicians. 
  • $1,488 subtotal. 

Total annual fee: $3,672.
