NCQA Online Application Process

The online application and scheduling process for organizations seeking NCQA Accreditation, Certification or Distinction applies to these products.

The process uses a web-based application tool—no need to download and fill out an application.

Organizations with current NCQA Accreditation, Certification or Distinction in a program listed above can apply for a Renewal Survey at my.ncqa.org. Log in, navigate to the Accreditation and Certification Application Tool and click the link. Once you are in the application, review and edit the prepopulated application information and submit the application directly to NCQA.
