About the On-Demand Training
This program will be an expert panel discussion on Measuring Quality–choosing and using measures, and understanding medical evidence: where it comes from, how it affects the practice, and where it fits in the standards. Quality measures have an impact in many of the NCQA standards and offer practices the opportunity to focus and coordinate their work. Measures will take on even more importance as practices prepare for value-based payment and MACRA.
Today, we find that most practices are new to using measures and don’t necessarily see the connections among several PCMH and PCSP elements:
• Quality Measurement and Performance Improvement
• Population Management
• Care Management
• Evidence-Based Medicine
• Standing Orders
This webinar will use specific examples to make connections among the evidence, the guidelines, the measures, and the use of measures in patient-centered care. Participants will be asked for other examples, and for questions they have about measures.
Objectives
After completing this on-demand training, you will be able to:
• Evaluate and choose measures for use in multiple, central PCMH functions.
• Focus efforts toward PCMH transformation to address specific clinical and utilization objectives through multiple NCQA standards using relevant measurement.
