Patient navigation is one of the fastest growing healthcare professions in the United States. This role is expected to grow even further as healthcare payment reform evolves. Patient Navigators have been around for nearly twenty-five years; however only recently have medical practices begun to realize the value navigators contribute to the Triple Aim of improved health outcomes, lower cost of care, and better patient experience. Patient navigation is not a “one size fits all” model of care. Therefore, practices must identify the effective utilization of the navigator in their practice.
This webinar is designed to help participants develop and enhance the role of the patient navigator that adds value and support to their practice operationally as a PCMH.
