​​​​​​NCQA Performance Measurement and Quality Improvement is the sixth foundational concept in the PCMH recognition program. This course is designed to aid in the transformation of a health center into a patient centered medical home with a culture of continuous quality improvement at its core.  Upon completion of this course a health center will be positioned to successfully meet NCQA’s application requirements for Performance Measurement and Quality Improvement and they will have the skills to sustain their quality improvement program and transformation.

At the conclusion of the program, participants will have the knowledge they need to:

  • Identify the basic concepts of QI.
  • Develop and/or refine your practice’s QI program for sustainability.
  • Assess and analyze the requirements for Performance Measurement and Quality Improvement

Faculty

Wanda Ali-Matlock, RN, MBA, FAACM, PCMH CCE

Wanda Ali is a Registered Nurse with 38 years of healthcare experience. She is the owner and founder of WAM’S Healthcare Consultants, LLC, a business she established in 2010 to meet the needs of families struggling to navigate the complex healthcare system. In addition, to managing her business, Wanda works full time at Medical Mutual of Ohio where she serves as a Sr. Clinical and Compliance Accreditation Specialist.  In this role, Wanda oversees the Health Plan Accreditation application process, training and compliance for the organization.

Wanda has a Bachelor’s Degree in Healthcare Management and a Master’s Degree in Business Management.  Wanda is a certified Case Manager and a Fellow in The American Academy of Case Management where she serves as a course writer and instructor.  Wanda is certified as a Patient Centered Medical Home (PCMH) Content Expert by the National Committee of Quality Assurance (NCQA) and she has supported several practices to achieve PCMH recognition.  Wanda also, has served as a NCQA PCMH Reviewer, a course writer and presenter at NCQA’s national conference.

Wanda previously, worked as a Senior Consultant, for Better Health Partnership, a Regional Healthcare Improvement Collaborative in Northeast Ohio. In this role she was involved in several grant funded initiatives such as hypertension improvement in African Americans, childhood obesity and asthma and she recently completed the preliminary work for the establishment of a Pathway Community HUB model of care in Cuyahoga County, which is a model of care that use Community Health Workers (CHW) to address Infant Mortality. Wanda’s experience in the CHW’s space is that of a researcher, curriculum and course developer and as a faculty for the Community Health Worker program at Cuyahoga Community College. 

Wanda resides in Cleveland, Ohio and in her spare time she enjoys vegan cooking, dancing and spending time with her family and friends.

Continuing Education

In support of improving patient care, the National Committee for Quality Assurance is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC) to provide Interprofessional Continuing Education for the healthcare team. 

This activity was planned by and for the healthcare team, and learners will receive 3.0 Interprofessional Continuing Education (IPCE) credit for learning and change.

This educational activity is approved for 3.0 nursing contact hours.*

This program grants 3.0 Continuing Education Unit (CEU) points for PCMH Certified Content Experts.  This program is not considered a required CCE quarterly webinar.

ATTENTION: Effective February 1, 2022, this on-demand (enduring material) activity no longer offers credits for the American Academy of Physician Assistants (AAPA), the American Psychological Association (APA), or the Association of Social Work Boards (ASWB). Only applicable professionals who fully completed this on-demand activity prior to February 1, 2022, are eligible for AAPA, APA, or ASWB credits. No other CEUs are impacted by this change.
