PCMH Recognition Annual Reporting: Continued Success OnDemand

This webinar is designed to help PCMH practices understand the larger goals of Annual Reporting and prepare for annual submission (for reporting years 2020 and 2021) under the Patient-Centered Medical Home program. Expectations for those transitioning from the 2014 PCMH recognition as well as those that have achieved Recognition under the current PCMH requirements will be discussed.

Faculty will highlight ways to demonstrate sustained recognition status through maintenance and continuous quality improvement efforts using data that a practice is already collecting internally and for other sources (e.g. state initiatives). Timelines for deliverables and performance expectations will be key discussion items.

What You Will Learn

  • Outline the Annual Reporting requirements for PCMH recognition.
  • Describe the Annual Reporting process; including timelines, submission and audit.
  • Interpret Annual Reporting requirements for reporting year 2020.
  • Recognize special topics in Annual Reporting and PCMH distinctions.

Who Should Attend

This webinar is geared towards individuals with one year of experience, or more; collecting, reporting, and acting upon annual reporting data; or those who need a refresher. This program is designed for individuals in the practice that work with patients, and with PCMH recognition.
