NCQA’s Advanced PCMH Program: Mastering the Medical Home Transformation

This advanced course is intended for individuals who have applied NCQA PCMH Recognition requirements in the practice setting. It examines the PCMH standards and the assessment process. Participants will take a deeper dive to each PCMH Concept and explore the key characteristics of a successful medical home in this hands-on program.

Faculty and participants will discuss evidence to support the core and elective criteria. The program also focuses on advanced topics such as the Distinction Modules, transfer credit and program benefits.  Attendees will participate in facilitated activities that demonstrate how earning elective credits builds upon the Core criteria of a Competency. Our faculty will incorporate topics on behavioral health integration, the importance of assessing the social determinants of patient health, engaging with patients and the community, and continuous quality improvement.

What You Will Learn

At the conclusion of the program, participants will have the knowledge they need to:

  • Discuss transfer credit and distinctions.
  • Explain how elective criteria build upon core criteria within a competency.
  • Examine opportunities and approaches to succeed in the PCMH model of care. 
  • Determine a strategy for preparing a practice for the three (3) virtual check-ins and begin to create a transformation plan.
  • Describe the types of evidence a practice can provide to meet the intent of a criterion, competency, and concept.

Who Should Attend

This interactive program is geared towards individuals with significant knowledge of the NCQA PCMH Standards and experience implementing those standards in a practice setting and who have attended the Introduction to PCMH 2017 seminar; and those who intend to obtain the individual Patient-Centered Medical Home (PCMH) Content Expert Certification credential.

Faculty

Nicole Mason, MA, PCMH CCE

Nicole Mason is an Assistant Director in NCQA’s Product Development Department. In this role, she oversees new product development and updates to NCQA’s Recognition Programs. Prior to this role, she was a Policy Manager in the Recognition Programs Policy and Resources Department, where she supported multiple aspects of the recognition programs, including development, implementation, maintenance, customer support and education. She started at NCQA in 2013 as an analyst in the Performance Measurement Department where she supported government contracts to develop electronic clinical quality measures, HEDIS measure maintenance and grant-funded research projects.

Before joining NCQA, she worked as a post-baccalaureate fellow at the National Heart, Lung, and Blood Institute. She holds a BA in Biochemistry and Molecular Biology, BA in Psychology, and MA in Biotechnology from Boston University in Boston, MA.

Amy Tennant, PCMH CCE

Amy Tennant is a Policy Manager in NCQA’s Recognition Programs Policy and Resources. In this role, she creates and maintains PCMH resources, manages product updates and provides policy support and expertise to customers, PCMH CCEs and NCQA Evaluators.

She started at NCQA in the Product Development department where she supported the development of recognition programs including the redesign of the PCMH standards for 2017, Patient-Centered Specialty Practice School-Based Medical Home and the creation of the Oncology Medical Home, the first recognition program created for a specific medical specialty.

Before starting her career at NCQA, she was an intern at MedStar Georgetown University Hospital where she worked on reducing medication reconciliation errors and incidence of outpatient falls. She obtained her Six Sigma Black Belt and holds a BS in Healthcare Management and Policy from Georgetown University in Washington, DC.

 

Continuing Education

In support of improving patient care, the National Committee for Quality Assurance is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC), the American Academy of Physician Assistants (AAPA), the American Psychological Association (APA), and the Association of Social Work Boards (ASWB) to provide Interprofessional Continuing Education for the healthcare team. 

This activity was planned by and for the healthcare team, and learners will receive 4.75 Interprofessional Continuing Education (IPCE) credit for learning and change. 

This educational activity is approved for: 4.75 AMA PRA Category 1 CreditTMANA CNE, APA, ACPE and ASWB ACE.

The assigned universal program number: JA0004597-0000-20-003-L04-P. Upon successful completion of this program (attending the full session and completing a program evaluation), participants will access CPE Monitor on the ACPE website to locate and track their CPE statement of credit.

This live seminar grants 6.0 Continuing Education Unit (CEU) points for PCMH Certified Content Experts.
