CCE Quarterly: First Comes Transformation! Innovative Ideas for Facilitating PCMH Recognition
This activity is designed for PCMH Certified Content Experts as well as staff members of primary care and specialty care practices who would like to learn innovative ways to garner resources through collaboration with local organizations to support PCMH transformation. Also, we will engage the learner and share success stories about the unique ways an organization has used groundbreaking techniques to support practices by integrating their EHRs and utilizing the data to identify and track patient populations who need higher levels of care.
Objectives:
At the conclusion of this educational activity, participants will be able to:
- Identify opportunities where collaboration with local colleges and universities can be developed to support local practices in their PCMH recognition.
- Apply innovative techniques that can improve the technological use and function of a practice’s EHR to track and monitor their most at-risk patient populations.
- Develop methods for engaging practices in a consultative capacity.
NOTE: This is a Quarterly Webinar for CCEs: This activity provides maintenance of certification credit (2 points) for PCMH CCEs.
