How to Effect Change as a PCMH Certified Content Expert

The purpose of the PCMH Content Expert Certification is to highlight one’s comprehensive knowledge of the requirements, the application process and the documentation of the NCQA PCMH Recognition Program. However, being an effective PCMH Certified Content Expert (CCE) goes well beyond passing the NCQA Certified Content Expert exam. 

With the increasing number of PCMH practices and PCMH CCEs throughout the country, there is a need to connect the certification with actual transformation guidance skills.  The practices and the CCE should not be able to just merely check the box and “take the test” but to achieve true transformation.  The ability to transform is the most important aspect of being recognized as a patient-centered medical home.

This session will discuss key skills, best practices and obstacles of guiding a practice through PCMH transformation.  We will answer the questions, what are the primary roles of CCE as a practice transformation advisor?  How do I become a trusted and valuable resource?

This educational activity is designed for PCMH Certified Content Experts as well as staff members at primary care and specialty care practices.

 

Learning Objectives:

 At the conclusion of this educational activity, participants will be able to:

 

  •         Identify key skills required to guide practices in achieving lasting transformation.
  •         Apply best practices to engage practices as a reliable resource for practices seeking PCMH recognition.
  •         Recognize potential obstacles, risks, and mitigation strategies when guiding practices through the recognition process.
  •         Discuss various professional options and ways to bolster experience for a PCMH CCE in a practice setting.

 As a PCMH CCE, this webinar provides maintenance of certification credit (2 points).  

This is a non CME/CNE activity.
