Team Based Care—It Takes a Village to Transform a Medical Home

 

Implementing care teams is a critical element of transforming a practice into a Patient-Centered Medical Home. NCQA focuses on a team-based model of care that enables a focus on the patient and family.

Defining teams and identifying consistent training and communication processes help practice team members recognize opportunities for patient and family centered whole-person care.

During this webinar, we will explain the meaning of team-based care and how you can create a well-functioning team. This will include outlining a process to assess current responsibilities and determine the various roles of the care team. We will review communication strategies, learn techniques to train staff to maximize capability and discuss ongoing performance measurement through quality improvement activities that involve the practice team, patients, and their families.

This activity is designed for PCMH Certified Content Experts as well as staff members at primary care and specialty care practices.

Objectives:

At the conclusion of this educational activity, participants will be able to:

  • Assess the current team based care model in a practice in relationship to the NCQA Patient-Centered Medical Home 2014 Standards.

  • Utilize and develop tools to facilitate and track ongoing training around patient population management, self-efficacy and behavior change, and care coordination.

  •  Improve communication techniques within the team and practice as a whole.

  • List, then prioritize the needs of a practice to assist the practice in achieving PCMH Recognition through a team based model of care focused on the patient and family. 

 

As a PCMH CCE, this webinar provides maintenance of certification credit (2 points). 
This is a non CME/CNE activity.
