About the Seminar
This highly anticipated program has been designed to help PCMH practices understand the larger goals of Annual Reporting and prepare for submission under the redesigned Patient-Centered Medical Home program. An examination of the annual reporting requirements will be discussed including expectations for those transitioning from 2014 recognition as well as those achieving recognition under the 2017 requirements.
Faculty will explore ways to demonstrate sustained recognition status through continuous quality improvement efforts and the use of data that the practice is already collecting internally and for other sources (e.g. state initiatives). Timelines for deliverables and performance expectations will be key discussion items.

At the conclusion of the program, participants will have the knowledge they need to:
• Outline the annual reporting requirements for PCMH recognition.
• Discuss ways in which annual reporting leads to continuous quality improvement within a PCMH practice.
• Describe the annual reporting process; including timelines, audit and submission.
• Outline important functions of QPASS, the annual reporting system.
