Join us for a live webinar designed to share the upcoming Annual Reporting Requirements for reporting year 2023. This webinar will help PCMH practices understand the larger goals of Annual Reporting and prepare for annual submission under the Patient-Centered Medical Home program.

Faculty will highlight ways to demonstrate sustained recognition status through maintenance and continuous quality improvement efforts using data that a practice is already collecting internally and for other sources (e.g., state initiatives). Timelines for deliverables and performance expectations will be key discussion items.

  • Describe the Annual Reporting process; including timelines, submission and audit.
  • Interpret Annual Reporting requirements for reporting year 2023.

This webinar is geared towards individuals with one year of experience, or more; collecting, reporting, and acting upon annual reporting data; or those who need a refresher. This program is designed for individuals in the practice that work with patients, and with PCMH recognition.

Faculty

Jennifer Anglin, MS, CHES, PCMH CCE

Jennifer Anglin is a Manager of Recognition Policy at NCQA. Her responsibilities include creation and maintenance of PCMH resources and providing support to practices going through transformation or annual review. Prior to NCQA, Jennifer worked for Purdue University by assisting practices throughout the state of Indiana on quality improvement and research projects as well as supporting them to meet Medicaid's Promoting Interoperability program and Medicare's MIPS program. Jennifer's professional interests include health policy,, research, and education. She has a particular interest in social determinants of health and the behavioral health aspect of Patient-Centered Medical Homes.

Jennifer holds a BA in secondary education from Valparaiso University and a master's degree in health science from Indiana State University. Jennifer is a Certified Health Education Specialist and has been an NCQA Patient-Centered Medical Home Certified Content Expert (PCMH CCE) since 2013.

Tracelyn Thigpen, RN, MSN, PCMH CCE

Tracelyn is a Senior Operations Engineer with NCQA’s Quality and Process Improvement unit and has been with NCQA since 2014. In her current role, she plans, leads and executes user acceptance testing and trains internally and externally on the Q-PASS system. She also works on process improvement projects to streamline business workflows.

Tracelyn attended Howard University in Washington, D.C. and received her Bachelor of Science in Nursing. She received her Master of Science in Nursing from Yale University in New Haven, CT.

Prior to joining NCQA she worked as a floor nurse on a Medical Surgical pulmonary unit. She has also worked as clinical faculty with a nursing school in Northern, VA.

Fatin Yousufzai, MSHP

Fatin Yousufzai is a Manager of Recognition Policy at NCQA. Her responsibilities include the creation and maintenance of PCMH resources and providing support to practices going through transformation or annual review. Prior to becoming a manager in Recognition Policy, Fatin was a Policy Analyst on the Performance Measurement team at NCQA, where she supported HEDIS measure development, maintained derivative HEDIS publications, and produced HEDIS Value Set Directories (VSDs).

This year she celebrates her 3rd year at NCQA. Fatin earned her B.S. in Health Sciences from Howard University and M.S. in Health Policy from George Mason University.
