Population Health Management as a Tool to Improve Hypertension and Diabetes in the Ambulatory Care Setting

Focusing on the health of entire populations is important to the advancement of both medical care and research. The clinical pharmacy team at the Southwest Community Health Center in Butte, Montana, implemented a population health management protocol to identify and provide resources for patients with hypertension and diabetes. The goal is to improve the quality of care, improve outcomes and reduce health care costs these patients. Similar programming could be extrapolated to additional patient populations.

Faculty

Kate McGree, PharmD, BCGP, CPP

Kate McGree is a Clinical Pharmacist at Southwest Montana Community Health Center in Butte, MT. She earned her Doctor of Pharmacy degree from the University of Montana, graduating with honors in 2005. Kate worked as a community pharmacist for 11 years before joining the SWMTCHC Clinical Pharmacy team. She is Board Certified in Geriatric Pharmacy and is a licensed Clinical Pharmacist Practitioner in the state of Montana. Her practice interests include geriatrics, COPD/Asthma, Diabetes, Anticoagulation, and Hepatitis C. She is also a Clinical Assistant Professor for the University of Montana School of Pharmacy.

 

Tammy Cox, PharmD, BCACP, CPP

Tammy Cox is the Director of Pharmacy Services at Southwest Montana Community Health Center. She graduated from the University of Montana School of Pharmacy and is Board Certified in Ambulatory Care Pharmacy. She is licensed as a Clinical Pharmacist Practitioner in Montana. She oversees all pharmacy operations in our federally qualified health center which includes our Clinical Pharmacy Program in 3 medical clinics as well as dispensing pharmacy services in 3 pharmacies. Tammy also manages the 340B program and medication use processes at each site. Her passion has always been providing clinical pharmacy services in a team-based care environment. She enjoys working 1 on 1 with patients and with the medical teams in order to help patients achieve their health goals and live their best lives.
